Please choose the type of transcript you are requesting—either Marriage or Baptism— and submit a request via the online form provided below.

*Processing fees apply: a standard $50 fee for non-stewards with an additional $100 fee required for Metropolis Verification, which is necessary for Greek citizenship purpose.
Once your request is submitted, the Assumption Office will review your form and will contact you for further instructions.
